Will My Appointment Be Covered?
Your appointment will not be confirmed until insurance has been verified. Insurance coverage is not guaranteed, please note that there may be a copay or deductible that has to be met depending on your insurance plan.
Medicare currently only covers:
Chronic kidney disease (not on dialysis)
Post-kidney transplant
Diabetes (not prediabetes)
If you meet the above criteria, you may follow the steps below to schedule your appointment! If you have Medicare, but would still like to see a dietitian, your appointment will need to be paid out of pocket. Good news! We do accept HSA/FSA payments.
Step 1: Have Your Provider Send a Referral Form (Required)
A) Our office is happy to request a referral on your behalf, or
B) you may download the form and give it to your doctor's office.

Step 2: Once referral is received, you will be contacted
to schedule an appointment.
Please make sure to complete the forms that are emailed to you in a timely manner.
